Your providers will give you the convenient, quality care you need all in one place – in-person or virtually. They are also experts in coordinating care, which means they can refer you to specialists when needed and make sure nothing is overlooked.

Primary Care

We offer a comprehensive range of primary care services that focus on your long-term health and preventive care. Your primary care provider will get to know you and your family so they are better equipped to prevent, treat and manage any health issues that may come up. The best part? Eating well can improve your overall well-being. It can also help treat existing conditions and decrease future risk. At Premise Health Wellness Center, your care team use medical nutrition therapy to help treat a range of conditions, such as diabetes, high blood pressure and heart disease, as well as many digestive disorders and weight-related issues. They will tailor your nutrition counseling to meet you and your family’s unique needs and lifestyle with personalized action plans, based on your risk factors and family history.

Our care team can provide advice and ongoing education virtually through one-on-one tailored sessions and individual nutrition assessments, group nutrition classes, lunch and learns, seminars and self-guided nutrition tools  to give you and your family a new appreciation for healthy food, and in turn, improve your overall health.

Be Well – Chronic condition management

The Premise Health Wellness Center offers a comprehensive approach toward wellness, preventive care and chronic conditions, where the focus is on you — your health and what it takes to have optimal health and maintain it. We offer the Be Well program to help you manage, prevent, and even reverse chronic conditions including high blood pressure, Diabetes, Asthma, COPD and Dyslipidemia (High Lipid Levels).

If your medical condition requires the involvement of a specialist, we will make referrals and continue to assist with coordination of care as needed for our members and patients. We also coordinate with other departments to make sure you receive the optimum care needed for your overall health. In addition, we offer integration wellness coaching to help you feel empowered to manage your health and wellness journey. Participants of the Be Well programs will receive preventive medication at no out-of-pocket cost (covered 100%).

Wellness coaching

Premise Health Wellness Center offers wellness coaching to focus on your overall well-being. Our certified wellness coaches will meet you where you are and help you make actionable goals to build new skills and make positive lifestyle changes. All coaching sessions are completely confidential and are available virtually.

Our wellness coaches address a variety of lifestyle needs, including weight management, physical activity, exercise, smoking cessation, work/life balance, stress management, and life satisfaction. 

During your initial coaching session, your wellness coach or dietitian will help you assess your readiness for behavior change, collaborate to create a wellness individual wellness plan, discuss your motivators and strengths, identify potential barriers to the changes you want to make and set long- and short-term goals.

During your follow-up coaching sessions, your wellness coach will help you: celebrate successes, discuss challenges and generate alternative strategies, commit to maintain or modify existing goals, measure progress toward long-term goals and revisit your wellness vision.

Lab services

To support you and your family’s needs, we offer clinical lab services that include onsite testing and specimen collections for offsite processing. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health patient portal or during an in-person or virtual follow-up appointment with your provider.

Behavioral health

At Shaw Family Health center we believe that your mental and emotional well-being are just as important as your physical health. With our behavioral health services, you and your eligible dependents can get the support you need. Your behavioral health specialists actively work as part of your care team to support your overall health while making you feel safe and comfortable. You can conveniently reach them in-person or virtually, through video or by phone.*

With a blended approach of counseling techniques and evidence-based talk therapy, our behavioral health counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues. *To conduct a local virtual visit, you must be in the same state in which your provider is licensed.

Provider Dispensing

Skip the extra trip to the pharmacy. You can get your medications for $7.00 per 30-day supply directly from your provider during a visit. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.


Shaw Family Health Center carries close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider.
