Virtual Primary Care

You don’t have to coordinate your own care. With virtual primary care through Premise Health, you have access to a dedicated provider by phone or video. It’s just like seeing a primary care provider (PCP) in person, but without the waiting room, commute or disruption to your day.

Skip the various appointments at different offices and trying to communicate with multiple providers. Instead, designate one of our virtual primary care providers as your PCP. They’ll become your trusted health ally and help you better prioritize your health.

Convenience

You can receive the treatment you need for an illness, get a medication refill, discuss test results, develop a wellness plan and more with your provider. You can contact your care team 24 hours a day, no matter where you are in the U.S.

Local Virtual Health

Connect with the same providers you know and trust during Shaw Family Health Center’s regular hours and continue to receive the highest quality care from the comfort of home. Local Virtual appointments are:

  • Conveniently scheduled. Available during your center’s regular hours. Select a time that works best with your schedule.

  • Familiar and trusted. With the same provider you know and trust at Shaw Family Health Center and Premise Health Wellness Centers.

  • Affordable: Anyone eligible to use the center can schedule a virtual appointment, and the cost is the same as an in-person appointment.

  • Good for ongoing care. Many conditions can be treated virtually – check with your center. Virtual appointments are also good for follow-ups or to review test results.

  • For in-state care only. To conduct a local virtual appointment, you must be in the same state in which your provider is licensed.

Virtual Counseling

Forget the commute to an office and the often-difficult referral process of finding a counselor. Instead, you can now schedule and conduct virtual counseling appointments by video or phone. No referral needed.

Associate Sign-up

  1. Visit My Premise Health online or through the mobile app.
  2. Click on “SIGN UP NOW”
  3. You can self-sign up (using your legal name) by verifying the last four digits of your social security number (SSN) and date of birth.

Dependent Sign-up:

  1. Complete registration at the time of your next appointment; or
  2. Email myphsupport@premisehealth.com and provide a phone number for a return call. Available Monday – Friday, 6 a.m. – 7 p.m. CST; or
  3. Visit or call the Center to receive a personalized registration invitation via email or text. Available Monday – Friday, 6 a.m. – 6 p.m. EST.
  4. Once you have created an account online, you can download and log in to the mobile app.
