Here are the virtual options available to you: 

Phone or video appointments with an onsite provider during your center's regular business hours

Connect with the same providers you know and trust during Shaw Family Health Center’s regular hours and continue to receive the highest quality care from the comfort of home. Local Virtual appointments are:

Best for: Those who want to see their local provider without traveling to the health center.

These virtual visits are:

  • Conveniently scheduled. Available during your center’s regular hours. Select a time that works best with your schedule.
  • Familiar and trusted. With the same provider you know and trust at Shaw Family Health Center or Premise Health Wellness Centers.
  • Affordable: Anyone eligible to use the center can schedule a virtual appointment, and the cost is the same as an in-person appointment.
  • Good for ongoing care. Many conditions can be treated virtually – check with your center. Virtual appointments are also good for follow-ups or to review test results.
  • For in-state care only. To conduct a virtual visit with your onsite provider, you must be located in the same state in which your provider is licensed.

24/7 access to care anytime, anywhere

You have on-demand access to a team of virtual providers available 24/7. These providers are all connected through the My Premise Health app, which means they have access to your health history and can easily communicate with your other Premise Health providers.

Best for: When you or a family member need to see a provider after hours or while traveling away from home.

Ongoing care with a designated virtual primary care provider (PCP)

With virtual primary care through Premise Health, you have convenient phone and video access to a dedicated provider for your preventive and ongoing care. It’s just like seeing a primary care provider (PCP) in person, but without the waiting room, commute or disruption to your day.
Designate one of our virtual providers as your PCP and they’ll become your trusted health ally to help support you along your wellness journey.

Best for: Members who prefer to receive care in a virtual setting with the same dedicated provider for every visit.

Comfortable and confidential visits with a virtual behavioral health provider

Our team of counselors can provide appointment-based support across all 50 states, giving you the flexibility to talk to someone from the comfort and privacy of your own home, wherever you are in the U.S.

Our secure member portal makes it easy to connect with your care team. Whether you’re at home or on the go, you can schedule appointments, message your providers, view your lab results and more – anytime, anywhere.
Associates can verify their information and sign up directly through the My Premise Health website or mobile app. Simply select “Sign up now” to get started. Dependents will need to visit or call the wellness center or email myphsupport@premisehealth.com to request a registration link.
