Primary Care

We offer a comprehensive range of primary care services that focus on your long-term health, including sick and preventive care and management of your chronic conditions. Your primary care provider will get to know you so they are better equipped to prevent, treat and manage any health issues that may come up.

Our lifestyle medicine approach ensures we’re looking at the whole picture of what impacts your wellbeing, including sleep, nutrition, stress, movement and more. We can encourage healthy habits to help you achieve sustainable wellness and go beyond simply treating the symptoms you bring with you to a single appointment.

Be Well Program

For the treatment and management of health conditions

You don’t have to navigate your health challenges alone. Our Be Well Program provides a registered nurse as your care manager to be a go-to resource for all health-related questions, prescription guidance and more. They will come up with a personalized treatment plan to help you make positive lifestyle changes to prevent, improve, manage or even reverse certain health issues.
Our treatment plans are tailored to meet your unique needs and can address a range of conditions such as high blood pressure, diabetes, asthma, COPD and dyslipidemia (high lipid levels). We can provide one-on-one coaching, disease-specific education, self-care training, nutrition counseling, medication guidance and ongoing monitoring to manage your symptoms and help you stay well.

If enrolled in a Shaw medical plan, you can also receive select preventive medication at no out-of-pocket cost (covered 100%) through the Be Well Program.

Behavioral health

We believe that your mental and emotional wellbeing are just as important as your physical health. With our mental health services, you can get the support you need. Our licensed clinical social worker actively works as part of your care team to support your overall health, while making you feel safe and comfortable. You can conveniently meet with them in person or virtually, through video or by phone.
With a blended approach of counseling techniques and evidence-based talk therapy, our counseling services cover anxiety, depression, stress management, PTSD, ADHD, grief, adjustment challenges, substance abuse, work/life balance, family conflict and sleep issues.

Provider-dispensed medications

Skip the extra trip to the pharmacy. You can get your medications directly from your provider during a visit for as little as $7 per 30-day supply. Simply schedule an appointment, discuss your health needs and leave with your medication in hand.
We carry close to 200 medications to cover your needs. This includes over-the-counter options. You can even get up to a 90-day supply for ongoing prescriptions to reduce the number of times you need a refill. When you’re about to run out, schedule a follow-up visit with your provider. They’ll monitor your progress and offer ongoing support.

Physical therapy

You don’t have to live with pain and discomfort. Our physical therapists are dedicated to your wellbeing. They work with your primary care provider to understand your specific health needs and conditions. They’ll get to the root cause of your injury and work with you to prevent future complications, so you can get back to moving with ease.
We offer a full range of in-person and virtual services, including therapeutic treatment for sports-related injuries, neck or back pain, and arthritic diseases and conditions; as well as hip, knee, shoulder, foot and ankle rehabilitation, and postural or ergonomic retraining.

Lab services

We offer many lab services to support your needs. We can perform onsite testing and can also collect and send samples to an offsite lab. Our turnaround time for processing routine lab tests is typically 24 to 48 hours. You can receive your results electronically through your My Premise Health account or during an in-person or virtual follow-up appointment with your provider.

*Lab orders placed by a Shaw Family Health Center provider are $3 per lab test. This does not apply for community-based provider ordered labs.  For labs ordered by other providers, we are not able to provide an estimate of these costs – please contact your insurance carrier or Lab Corp for more information on estimated costs.

Vaccines

Vaccines are an essential part of public health. You have convenient access to many vaccines at Shaw Family Health Center. Your provider can determine which ones are right for you based on your age, conditions, travel habits and other factors.
We offer a variety of routine vaccines to protect your health and community, including:
  • Influenza (Flu)
  • Hepatitis A
  • Hepatitis B
  • Human Papilloma Virus (HPV)
  • COVID-19
  • Measles, Mumps and Rubella (MMR)
  • Pneumococcal (Pneumonia, Meningitis, etc.)
  • Tdap/Td (Tetanus, Diphtheria and Pertussis / Whooping Cough)
  • Varicella (Chickenpox)
  • Zoster (Shingles)

Wellness coaching

Wellness coaching can help you achieve lasting lifestyle change in the areas of weight management, nutrition, physical activity, sleep, stress, smoking cessation, work/life balance and life satisfaction. Our certified wellness coaches are trained specialists who support you as you move toward better health at a comfortable pace. They will help you set actionable goals and teach you new skills as you learn to incorporate healthier habits into your daily routine. All coaching sessions are completely confidential and can take place virtually or in person.

Nutrition

Eating well is crucial to maintaining your overall health. Our care team uses medical nutrition therapy to help treat or prevent a range of health conditions such as diabetes, high blood pressure and heart disease, as well as address many digestive disorders and weight-related issues. We will tailor your care to your individual needs and come up with personalized wellness plans that factor in your lifestyle, risk factors and family history.

We can provide advice and ongoing education through one-on-one sessions, individual nutrition assessments, group nutrition classes, lunch and learns, seminars, self-guided nutrition tools and virtual appointments. Our goal is to give you a new appreciation for nutritious food, and in turn, improve your overall health.

Care Consult

When it comes to your health, sometimes a second opinion is important. If you ever receive an abnormal test result or an unclear diagnosis, or have been told you need major surgery, our providers can virtually consult with the specialists at Mayo Clinic. Together, they will review your medical record and come up with a recommended treatment plan for your specific health concern. Your provider will then share the outcome of that conversation with you to help you navigate your care plan and any next steps.

Care Excellence

We make sure you’re never alone in making complicated decisions about your health. If you’re struggling to get a diagnosis or battling a rare, complex condition, we can help. Our partnership with Mayo Clinic offers you access to highly specialized care.
If medically appropriate, your provider will refer you to the Mayo Clinic for evaluation and/or specialized treatment. A Mayo specialist and your provider will work closely to coordinate your ongoing care.

Should you need to go to the Mayo Clinic, all travel and lodging are conveniently arranged for you and one caregiver. All costs associated with the Mayo Clinic Care Excellence program, including services, travel and/or lodging are covered for those enrolled in the Shaw Medical Plan. For those not covered by the Shaw Medical Plan treatment and/or travel expenses will be subject to the insurance carrier’s guidelines.

Occupational health

Keeping you safe at work is a top priority. Your providers at Shaw Family Health Center are here to treat work-related injuries and illnesses to help you get back to feeling well.
Our occupational health services are available for all Shaw associates and include injury diagnosis and treatment and musculoskeletal evaluations.
Available at Andalusia, Cartersville and Dalton locations.

Care Navigation

There may be times you need specialty care outside of our center. Finding that care on your own can sometimes be confusing. That’s why we offer care navigation services to support you in finding the affordable, high-quality care you need.

If your provider makes a specialist referral, our care navigation team will help you find the best care at the best price in your community. They’ll even assist with scheduling your appointments, transferring your medical records and following up after your visits to help with any ongoing care needs.
